New Hire Documents

All of the pre-employment documents mentioned below must be completed by new employees and former employees who are being rehired (best viewed in Adobe Acrobat Reader 5.0 or higher).

Step 1/5:

All employees, excluding affiliates & adjuncts, must provide their original forms of eligible identification during their onboarding as well as for Form I-9. 

Step 2/5:

(Skip if U.S. Citizen or Permanent Resident)

Foreign Nationals must have the following:

  • Original I-20 or DS-2019.
  • Copy of I-94
  • Current Passport
  • Any other employment authorization documents that may apply (i.e. EAD, H1B, etc.)

Step 3/5:

Complete the electronic Form I-9 by the first day of employment. To log in, use employer code 15799.

  3. Memorandum to Personnel File (N/A for Affiliates)
  4. Security Questionnaire
  5. Intellectual Property Agreement (Click here for the IP Agreement for Affiliates)

Financial Forms: (All are mandatory if you are being paid by Georgia Tech.)

  1. Federal Tax Form
  2. State Tax Forms (for non-Georgia withholding taxes)
  3. G-4 Tax Form (for Georgia withholding taxes )
  4. Direct Deposit
  5. Social Security (Tech Temps Only)

If the last day you worked at Georgia Tech was less than 24 months ago:

All rehires must confirm with their work department that they are aware of any previous Georgia Tech employment.
