Important 2016 FSA Activation Required

The Optum Health Savings Solutions website is mycdh.optum.com and requires account activation.

The University System of Georgia offers two great ways for you to set aside pre-tax dollars from your paycheck to reimburse you for qualified dependent day care and out-of-pocket healthcare (medical, pharmacy, dental, and vision) expenses through participation in a Flexible Spending Account (FSA). Optum is the FSA administrator for the USG.

Eligibility

Regular full-time/part-time faculty or staff who work at least 30 hours per week. 

Flexible Spending Accounts

A Medical FSA is an account that provides you, your spouse, and eligible dependents with pre-tax reimbursement for qualified healthcare (medical, pharmacy, dental, and vision) expenses that are not covered by insurance. 

Healthcare deductibles, co-insurance, co-pays, prescriptions, glasses, orthodontia, certain over-the-counter medicines, and supplies are eligible if incurred while you are participating in the plan. For a comprehensive list of qualified healthcare expenses, review the FSA eligible expense list. Optum will provide you with a debit card to pay for your qualified expenses.

If you have experienced a qualifying life event such as marriage, divorce, birth of a child, or your spouse's employment status has changed; you have 30 days to notify Georgia Tech Human Resources at 404-894-GTHR (4847) and provide proof of your qualifying life event. To make changes to your FSA as a result of one of these qualifying life events, complete the FSA Change Form and fax, Attention: Benefits, to 404-894-6978.

Dependent Day Care Spending Account

A Dependent Day Care Spending Account provides pre-tax reimbursement for your eligible dependents' childcare expenses. Under certain circumstances, the account may be used to help pay for the care of elderly dependents or a disabled spouse or dependent. Optum will provide you with a debit card to pay for your qualified expenses or you can set up Automatic Dependent Day Care Reimbursement with Optum.

Eligible dependent day care expenses may include services inside or outside your home by anyone other than your spouse, a relative, or a person you list as a dependent for income tax purpose or one of your children under the age of 19. Services may be provided at a child or adult care center, nursery, preschool, after school, or summer day camp. This care must be for:

  • a dependent child under the age of 13 years old, who lives with you for more than half the year and is supported by you;
  • a dependent child older than 13 or other relative who is incapable (disabled) of self-care and requires care while you are at work;
  • a non-relative you support who is incapable of self-support.

Effective Date

Enrollment in the Flexible Spending Account is effective the first day of the month after your enrollment is submitted.

Changes to Your Election

Due to restrictions imposed by the IRS code, after you make enrollment decisions during your initial eligibility or open enrollment, you cannot make changes during the remainder of the plan year (Jan. 1 to Dec. 31) unless a qualifying change in family status occurs. Please see the Family Status Change section for more information. At the time of your qualifying family change, you may not reduce your annual election to lower than what you have contributed year-to-date at the time of the change to your election. Processing Claims

You may use the Mastercard debit card provided to you by Optum or you can submit paper claims. For any claims paid using the debit card that are not co-pays under the medical, dental, or vision plans, you must substantiate proof of their eligibility within 60 days to Optum. You are required to file all paper claims for reimbursement within 90 days from the end of the plan year or within 90 days from the date you are no longer eligible to participate in the plan. You can file your claims by submitting the FSA Reimbursement Claim Form or by going online and providing your claims detail on the Optum website. You can also set up automatic Dependent Day Care Reimbursement for recurring dependent day care expenses.
